Help us learn whether this programme works

• Your five dental anxiety (MDAS) answers and their total, at each check-in.
• Structured answers about appointments booked and attended, steps you have managed, how your goal is going and how useful the programme has been.
• Structured before- and after-appointment answers, if you choose to use them.
• Your consent version, consent time and the date your baseline was recorded.

• Your name, email, address, postcode, phone number or date of birth.
• Your dentist's or practice's name.
• Anything you have written freely: diary entries, dentist plan, appointment reviews.
• The name or practice you may add to your printed report.
• Advertising or tracking data — there is none anywhere on this site.

Ordinary internet infrastructure still handles technical connection data in order to deliver the page. We do not use it as an evaluation field, and we will not pretend it does not exist.

• Now: your baseline anxiety answers are copied across when you join.
• Straight after your assessment: three short experience questions.
• Around four weeks: the anxiety questions again, plus what has happened.
• Around three months: the same again. This is the main check-in.
• Any time you like: before and after an appointment.

Prompts appear gently on your dashboard. There are no evaluation emails in this version, and you do not need an account.

You can delete every evaluation record linked to your code from this page, while the code stays on this device. If that local code is lost — for example if you clear this browser — we genuinely cannot tell which pseudonymous rows were yours, so we would not be able to delete them for you. That is the honest cost of not holding your identity.

This is an observational service evaluation, not a controlled trial. Even if scores improve, that cannot show the programme caused the change. Figures are only ever published as group totals with their denominators, and we never invent a threshold for a “clinically meaningful” MDAS change.
